I tested the pool barrier gate against AS 1926.1 and it failed to self-close and self-latch from one or more start positions, or I measured the latch release below 1500mm, or I passed a 100mm gauge beside or beneath the gate leaf. Sometimes the gate swings inward toward the pool rather than outward. Pool drowning remains the leading cause of accidental death for Australian children under five, and gate failure is the single most common finding I record at annual pool barrier inspections.

I inspected a stair flight of four or more risers (or any stair with an open side or a drop greater than 1m) and found no continuous handrail, or I measured the top-of-rail outside the 865-1000mm band I reference from NCC 3.9.2. I also note handrails that terminate short of the top or bottom nosing, flat-topped timber caps wider than 50mm I cannot grip from above, or rails removed during renovation and never reinstated. In my experience this defect is a leading contributor to domestic fall injuries I see, especially on external deck stairs and split-level entries.

I inspected a balustrade on a deck, balcony, or stair with a drop greater than 1m and found horizontal rails, lattice, or decorative infill a child could foothold, or I measured gaps between vertical members wider than 125mm. When I test the lower portion with a sphere gauge I find climbable surfaces in the zone 150-760mm above finished floor. Reference: NCC 3.9.2.5 (non-climbable zone in effect since 1996). I see this on both post-1996 builds and older homes that were rebalustraded without the rule being considered.

I inspected a fully glazed door or frameless glass panel where the pane has no contrasting visibility marking and can be mistaken for an open passage. Reference: AS 1288. I see this most often on frameless pivot doors, large stacker sliders, and floor-to-ceiling living room glazing in modern architectural homes. In my experience the injury pattern from walk-into incidents is forehead, nose, and wrist trauma, with children and elderly occupants disproportionately affected.

I identified annealed (non-safety) glass in a human impact location - most commonly a door, a side panel beside a door, a low sill window, or a shower or bath enclosure. The pane carries no Grade A safety stamp (no 'T', 'L', or compliance mark I could locate in the corner etchings). In my experience non-safety glass shatters into large shards on impact and is responsible for the majority of domestic glass-laceration injuries I see. Reference: AS 1288. Common in pre-1989 housing, unrenovated bathrooms, and owner-built extensions with salvaged glass.

I measured around the pool barrier and found objects, landscaping, or building features a child could use to scale the fence - inside the 900mm outside arc and/or the 300mm inside zone I check against AS 1926.1. Typical breaches I record include pot plants, pool pumps, retaining walls, trees, pergola posts, and outdoor furniture pushed against the fence line. Extremely common on established properties where landscaping has matured around an originally compliant barrier.
